Problem!!! Fan always ON

I hope someone can help me with this. Recently, I've noticed that my engine temperature gauge rarely reaches tne normal operating temperature. It usually takes a while for the temperature gauge to reach 1/4 of the way and than rarely would reach the 1/2 mark like it did before. What I've also start noticing is that the fan does not seem to ever turn off. It is always ON as soon as the car is started. Even whhen the temperature is still cold... the fan would come on. Is this normal? What can I do to diagnose this problem? Please help.

Could be a stuck thermostat, or maybe a bad temp sensor.
