Update: I got a new tensioner assembly installed and I was able to compare the old and new.
The old tensioner had about 1/2" of travel where there was no hydraulic damping, only spring tension. In some cases, as I ran it through its travel, it would be able to cycle through its full travel with no damping force, and then suddenly I would feel it sort of 'ratchet' upwards. Very odd.
The new tensioner has consistent damping throughout the entire travel. As a result, the system is much quieter now and while it still makes a noise it's about 1/4 of what it was before. I believe the noise is the small movements as the tensioner moves. 100k miles on this tensioner.
