Common issue with coilovers?

My M3 has ST coilovers and had this issue over the weekend. Is it common for the mount to break like this? Has anyone had a similar issue? Wondering if I replace with the same brand coilovers or different or if it was a mount issue and not the coil over itself.

There should be a big washer on the bottom side and a big rectangular washer/thing on top. It looks like those are missing.
